Adobe premier cc 2017 version render question

Essentially I'm trying to shorten a pre-existing video I recorded to meet a length/timing requirement. I built out a quick project, placed the clip in the edit line, shortened, added fade in/out transitions and went to render/export the video. It needs to meet the following requirement for format: Video must be shot in 4k resolution and QuickTime (.mov) or mp4 format, created with H.264 codec.

When I went to set the codec to h.264, the pixel dimensions max out at 2000x2000 and won't allow me to set it any higher. Any suggestions on proper settings to allow this codec and proper dimensions?

I see a preset option called "YouTube 2160p 4K". Do you have that one as an export preset. See attached picture for what I am looking at. That should export an H.264 at 4K easily.
